This is a tool for parents to use to rate their child’s inattentive, hyperactive, and impulsive behaviors. The Vanderbilt ADHD Diagnostic Parent Rating Scale was developed by Mark L Wolraich and is used in clinical settings to help diagnose and monitor ADHD treatment. Following the 18 questions, click view score. Look in the top right-hand corner. Point totals of 6 or higher suggest professional consultation is recommended.

This is a tool to help identify symptoms of anxiety in teens and adults ages 14 and older. Total scores of 6 or higher suggest professional consultation is warranted.

This is a tool to help identify symptoms of depression in adolescents and adults ages 12 and older. Total scores of 4 or higher suggest professional consultation is warranted.

Originally designed as a screener for bipolar disorder, this screening device is also used for anxiety, trauma, substance use, and impulse control disorders. For adolescents ages 12 and adults.
